Provide epilepsy education in schools and the general community. Respond to requests from schools, community agencies, and corporations to schedule presentations and/or training. Act as a community representative for the agency, promoting agency services.
Respond to and assess Information & Referral calls for potential service eligibilityIdentify gaps in services and, as appropriate, communicate with the Development department to collaborate on potential programs that meet consumer needs.Participate in regular agency intake meetings for case reviewEnsures consumer health and safety needs are addressed at all timesLinks and refers consumer to supports and servicesCompletes training to gain additional skills and meet agency, regulatory body, and standard of work requirementsOversee service delivery and reporting requirements for both Central and Broome DDSO FSS contracts. Act as staff liaison for contracted FSS programs through Central and Broome DDSOProvide epilepsy education in schools and the general community. Respond to requests from schools, community agencies, and corporations to schedule presentations and/or trainingRespond to requests from community agency/organizations and attend health fairs/community fairs, providing agency information, education and awarenessKeep up-to-date on current changes in the Epilepsy communityCoordinate events surrounding National Epilepsy Awareness Month in NovemberCoordinate and facilitate the Parent & Family Network Support Groups in service areasDevelop an article or program information for the quarterly newsletterParticipate in agency special events when possibleServe on agency wide committees as assignedComplete required documentation and reports – Quarterly contract reports, etc.Other duties as assigned
Education and Experience:
A Bachelor's Degree in a health and human services field and one year of experience providing information, linkages, and referrals regarding community based services for individuals with disabilities.Must possess and maintain a valid NYS Driver's License; a vehicle is required to perform the job.
Must be able to talk, hear, walk, twist, bend, use hand to finger; sit for extended periods of time.Specific vision abilities required by this job includeClose visionAbility to adjust focusAbility to view computer screens and use fingers for key board
Ability to train in and perform all required trainings for this position
While performing the duties of this position, the employee travels by automobile and will be exposed to changing weather conditions.
OSHA Category: This position requires tasks that involve exposure to blood or body fluids that requires specific training for the employee's safety. All Epilepsy-Pralid Inc. employees are offered the opportunity to receive Hepatitis B vacation series.
